Requirements to request an appointment:
- Appointment sheet issued by the Canadian Embassy, Medical Report (containing IME, ICU or EMR), School or Work.
- Original and a copy of a valid passport; if you do not have a passport, an original and a copy of a birth certificate.
- If you are taking medication or medical treatment, you must bring a letter from your doctor, your medication, and your prescription(s). It is not necessary to discontinue medication.
Requirements for your medical exam:
- Fasting is not required for children under 15; they can eat a light breakfast before the medical exam, such as juice, yogurt, or fruit (a small cup of fruit).
- Those over 15 years old fast for 6 hours, they can drink water.
- For women DO NOT PRESENT DURING THE MENSTRUAL CYCLE.
- If you wear glasses, it is important to have them present on the day of your medical exam.
- Minors under 18 must be accompanied by one of their parents or the petitioner. If one of the parents or the petitioner cannot attend, they must send a notarized power of attorney with the notary's seal and signature.
Before your appointment:
- Download and complete the "Personal Data Sheet" form which you can download here.
- Wear comfortable clothing, without accessories.
- If you have had surgery in the last 5 years or suffer from chronic illnesses, please bring a medical summary that is less than 3 months old (with diagnosis, treatment, and prognosis, on letterhead and in English or French). If you suffer from high blood pressure, please bring a recent electrocardiogram (less than 3 months old).
- We have free parking.
- We have special areas for our visitors, your companions can wait comfortably with the availability of coffee, food and drink machines and games for children to hang out.
IMPORTANT
During the visa application and interview process, you must be truthful and provide complete and accurate information. Failure to do so may result in a delay in your visa processing or in your being found ineligible for a visa that might otherwise have been approved.
